You may be at risk for prediabetes and type 2 diabetes if:
· You are 45 years old or older
· Type 2 diabetes runs in your family
· You are overweight or physically inactive
· You’ve had COVID-19

You’ll receive the program at no additional cost if you or eligible covered family members are enrolled in a King County medical plan, are at risk for type 2 diabetes or are living with diabetes, are 18 or older, and are accepted into the program.
